Rejuvené Family Medicine is proud to serve the needs of Baker County residents and the surrounding areas.

Our Services

We provide comprehensive primary care services for all ages, including:

  • Preventative medicine, health screenings, immunizations
  • On-site lab draws, and rapid testing for acute illnesses
  • DOT physicals, sports physicals, and pre-employment physicals by appointment only

We accept Medicare, Tricare, and most commercial insurances. For those without insurance, we offer affordable cash-pay services.

Our Story & Mission

Rejuvené Family Medicine was founded by Rachel Sirois, APRN, FNP-BC, who brings more than 12 years experience in healthcare – including nearly 3 years as a family nurse practitioner. Rachel is a proud graduate of Jacksonville University, and her first job as a nurse practitioner was right here in Macclenny at a convenient care clinic. It was during that time that she recognized a significant need: many patients lacked access to a primary care provider.

Rachel and her family quickly fell in love with the small-town feel of Macclenny, ultimately deciding to purchase land, build a house, and plant some roots. They officially became Baker County residents in January 2022, and shortly after, Rachel began planning what would become Rejuvené Family Medicine.

Her vision is to close the gap in access to primary care by offering a safe, welcoming, and Christ-like environment for all patients – regardless of insurance status. Whether you have Medicare, BCBS, or no insurance at all, our goal is to help make the Baker County community healthier!

I’m not sure who wrote this, but it was sent to me by my dad so I thought I would share.

Memorial Day should make you a little uncomfortable. Because sacrifice should never be comfortable. But from that discomfort can come gratitude. And from gratitude, a commitment to live better—not in guilt, but in honor.
Memorial Day is not a celebration—it’s a remembrance of both their sacrifice and the covenant left upon us. A day when the living should ask themselves one question: Am I living a life worthy of their sacrifice?
No one is asking you to feel guilty. They fought so you wouldn’t have to. But what they ask—what they hope—is that for one day out of the year, Americans pause to reflect on the cost of the freedoms they so casually enjoy. Reflect on the blessings of safety, security, and opportunity. Reflect on the cost paid by strangers so that your kids can play in the yard without fear.
Teach your kids about the meaning of the day. Toast to the fallen. Or simply take a moment of silence—real silence. That silence—that discomfort—is where gratitude begins. And from gratitude comes action.
This Memorial Day, enjoy your freedom. But don’t forget where it came from.
It’s not a holiday. It’s a headstone.

Hi RFM family! I wanted to let you know we hired another provider to help us with overflow. She is actually my very own sister! She will be seeing patients via telehealth only, and the only insurance she accepts at this time is BCBS and Medicare. We are working on adding her to others. If doing a follow-up or sick visit would be easier for you to do via telehealth, let us know! Her hours right now are Tuesdays, Wednesdays, Thursdays from 3:00pm to 5:00pm. These hours may change based on need.Erika Paskey is a dedicated Nurse Practitioner with a Doctor of Nursing Practice (DNP) degree and over two decades of experience in healthcare. Since beginning her career in 2002, she has been committed to providing patient-centered care with a strong focus on health promotion, disease prevention, and holistic well-being.Specializing in primary care with experience in gastroenterology, otolaryngology, rural health, and urology, Erika is passionate about improving access to skilled and knowledgeable healthcare for diverse patient populations. She believes in the power of a healthy lifestyle and works closely with patients to develop personalized wellness plans that support long-term health. Through shared decision-making, she ensures that her patients are actively engaged in their care, empowering them to make informed choices that align with their individual health goals.
With experience in both specialty and primary care settings, Erika integrates evidence-based practice with compassionate, individualized care. She is especially committed to serving rural communities, addressing healthcare disparities, and enhancing patient education.Beyond clinical practice, Erika is an advocate for preventive medicine, community health initiatives, and mentoring future healthcare providers. Her mission is to inspire and support individuals in achieving their best health through proactive, patient-centered care.
